"Play- Up" forms

Player Release Form
Parent/Guardian Release Form
"Playing up" requirements are outlined by our sanctioning organization USA Hockey Inline.  Any player who intends to participate in a division for which their chronological age does not fall between the age range of that division must complete these forms.   The player must request in writing consideration to "play up" as outlined in the USA Hockey Risk Management memo below.  Players AND guardian must read and sign the forms below.  The board of directors and head of referees will consider the written request.  Completion of the forms does not assure permission will be granted by the administration of the LCIHA.  The criteria for “PLAYIN UP” were established by USAHI - Risk Management Opinion, February 27, 2001, James Archibald, Risk Manager, USA Hockey Inline.

USA Hockey Inline Registration

All LCIHA participants must be members of USA Hockey Inline.  Registration will be the responsibility of the player, and will no longer be coordinated through the LCIHA.  Step-by-step directions have been prepared to assist you with online registration.

Players must demonstrate their membership through a photocopy of the registration form and check or the confirmation email that registration has been completed before being allowed to play hockey.

You can register with USA Hockey Inline on their website, or can download a hardcopy of the registration form from the site.

P.A.Y.S. Sportsmanship Training (schedule)

The Parent Association for Youth Sports (P.A.Y.S.) training is a mandatory parent education program held to inform parents on what is occurring in youth programs throughout the nation and how they can assist the league, coaches, or officials in making a better atmosphere for their children. At least one parent or legal guardian from each household must attend a P.A.Y.S. training session.  Call 541-2563 for more information.

P.A.Y.S. training is required for at least one parent/legal guardian of each player in the youth leagues (U10, U12, U14, and U17).

P.A.Y.S. is required even if parents/guardians do not attend the participant's games.

Certification is good for all city-sponsored sports programs for one calendar year.  Check the date on your card to determine when your certification expires.  The LCIHA will receive a list of those certified to determine who is eligible to play.

A parent/guardian must attend a P.A.Y.S. session before the second week of the season.  P.A.Y.S.  After the second week of the season, players whose parent/guardian has not attended P.A.Y.S. will not be allowed to participate until a parent/guardian has attended.

Late registrants will be afforded a two week grace period after their registration to have a parent/guardian attend a P.A.Y.S. session.  Any scheduling conflict with this requirement should be addressed with LCIHA League Manager.

A certification card is provided to P.A.Y.S. participants at the conclusion of each session.  This card is the only proof of attendance accepted by the LCIHA.

P.A.Y.S. Training is a requirement of the City of Las Cruces. The City of Las Cruces provides funding to the LCIHA based on compliance with the Resolution between the City and the LCIHA.
